Annual Summary of Work-Related Injuries and Illnesses

    California State law requires Administrators at every office and school to review, sign, and post the Annual Summary of Work-Related Injuries and Illnesses (Occupational Safety and Health Administration (OSHA) Form 300A). This form must be posted from February 1st until April 30th in a conspicuous place where notices to employees are customarily displayed.

    To access your site-specific report:

      1. Click on the link above "OSHA 300A LAUSD REPORT 2023" and download the Adobe PDF file.
      2. Scroll through the table of contents located in the left column by Cost Center Code minus the first digit. The Cost Center Code is a seven-digit number that typically starts with 1 or 9 and is followed by the Location Code. This report excludes the first digit of the site’s Cost Center Code, it will begin with the second digit. (Example: cost center code of 1234501 will be found under 234501).
      3. Select and print the reports for each cost center at your facility. Any cost center that is used for time reporting or payroll will have a separate report. For example, an elementary school that has a magnet program and infant center will need to print a total of three summary reports, one for each cost center. Be sure to print only those pages for your location, not the entire report.
      4. The site administrator needs to review the information, sign it at the bottom right corner and post it in a conspicuous place where notices to employees are customarily displayed from February 1 through April 30.

    Information for the 300A Summary was compiled from employee injury claims reported to Sedgwick Claims Management Services for 2023. It is a summary of all the OSHA recordable injuries by location. For additional information regarding this requirement, refer to REF-5693.4 Cal/OSHA Log of Injuries and Illnesses.

    The detailed report of workplace injuries, Cal/OSHA Form 300, is a confidential report that can be provided by request.  It lists each OSHA recordable injury individually including employee name, description of injury and lost work days.   Administrators may request a copy of the Cal/OSHA Form 300 to review detailed injury and illness data for their site.  This form is confidential and employee privacy must be maintained at all times.  Please contact OEHS at (213) 241-3199 to request a copy of the OSHA 300 Log for your location.

    *Site Administrators are required to verify the OSHA 300A log is posted in the Administrator’s Online Certification. This is due by January 31, 2024. You may refer to MEM-6128.12 Administrator Certification Online System 2023-2024 For School Sites and Offices
