Accident Prevention

OEHS is available to assist site administrators in determining the cause of workplace accidents and developing an action plan to reduce the frequency of worker injuries. Site administrators may request a Loss Analysis Report specific to a school, location, or Division, which can help to identify problem areas. Accident prevention training can be arranged by contacting OEHS at (213) 241-3199. All accidents and injuries must be investigated and reported within 24 hours of the incident. The District’s Injury / Accident Investigation Report has been incorporated into the Incident System Tracking Accountability Report (ISTAR). Serious employee injuries that result in death, disfigurement, dismemberment or hospitalization for more than 24 hours for treatment other than observation must be reported to the California Occupational Health and Safety Administration (Cal/OSHA) and OEHS within eight hours.
