    Steps to Obtain Evidence of Self-Insurance

     

    1. Download and fill out a Request for Proof of Self-Insurance.

    Tip: The Request for Proof of Self-Insurance is a fillable PDF. Download the file and fill it out on your computer (remember to save your changes).

     

    1. After you have completed filling out the Request of Proof of Self-Insurance, email the request to riskfinance@lausd.net.  

     

    1. Once you have submitted your Request for Proof of Self-Insurance, a representative from the Risk Management Department’s Insurance Compliance Unit will process your form and send a Letter of Self-Insurance to the requester and the certificate holder.
