Every attempt has been made to ensure accuracy of the 2023 W-2 statement information.  However, any employee who feels the information is inaccurate may complete a W-2 Inquiry Form, Attachment B of the 2023 W-2 Wage and Tax Statements, and submit the completed form to the Payroll Support Services office by fax at (866) 761-7413.  If the W-2 is inaccurate, an amended W-2 (W-2C) will be issued.
